Primary Care Billing Services
The management of the revenue cycle for longitudinal outpatient care. Primary Care Billing Services is an area where margins are slim and volumes are huge, and so a tendency to cut a few dollars from each claim will cost you far more than one denial will ever do. We code each claim according to the payer’s rules and thus ensure first pass acceptance while preventing revenue from leaking because of wrongly coded modifiers and add-ons.

Outsourcing Primary Care Billing To UControl
The outsourcing process gives the revenue cycle to a certified and HIPAA-compliant team that manages all steps starting from eligibility determination to final payment. UControl handles everything related to the process of patient eligibility determination, billing charges, CPT and ICD-10 code validation, modifier addition, claims filing electronically, payments posting, denials handling, and accounts receivable management regardless of practice sizes.
Primary care includes various services; each of them is coded in its way and has different requirements from the payer side. Annual wellness visit, preventive exams, chronic and transitional care management, immunizations, sick visit, minor procedure, and behavioral health screening.
Primary Care Physicians UControl Bills for
Family Practice Physicians
seeing patients at all ages within the scope of regular visits, chronic care management, and preventive services.
Pediatricians
who provide well-child visits, immunizations, and development assessments as per the appropriate age group.
Physicians
who provide direct primary care and bill separately from insurance companies.
Coding telehealth-enabled practices
with the right place of service and the insurance modifier.
Internal Medicine Physicians
who treat adult patients with complex and difficult-to-manage health problems.
Geriatricians
who deal with management of multiple diseases and complex Medicare documentation.
CHC/FQHCs
conducting high volume patient visits and billing program-sponsored services.
Coding practices
focused on chronic illness by charging for repeat CCM and PCM for diabetic, hypertensive, and cardiovascular diseases.
It is also important to mention that primary care includes women’s health screenings and walk-in visits that further differentiate the 25 modifier. These services in UControl practice are coded under the primary care revenue cycle and not considered separate service lines.
Preventive and wellness services
Preventive care is another path of reimbursement altogether from problem-oriented care. The Medicare program reimburses G0438 and G0439 codes for the Annual Wellness Visit (first-year visit and following visits). The code G0402 is reimbursed for the Initial Preventive Physical Examination during the first year of the Medicare Part B enrollment period. Codes 99381-99397 represent commercial preventive health services based on age of the patient. Counseling services have codes 99401-99404. If the patient mentions a new problem during the wellness visit, the E/M service associated with that problem is coded separately with modifier 25, and the payers review that combination with much scrutiny. The failure to differentiate between the two in documentation is what causes the revenue to collapse.
The preventive and problem services are differentiated at the documentation stage even before coding, and this helps us code the modifier 25 E/M service with the medical necessity requirement of the payers.
Immunizations, telehealth, and remote monitoring
Immunization revenue hinges on the link between diagnosis and procedure. Immunizations are submitted using 90460 and 90461, when there is counseling up to age 18, otherwise 90471-90474. Each code is denied in the absence of proper diagnosis coding. Telehealth brings another layer of place of service and insurance-specific modifiers. Remote Physiologic Monitoring brings new supply and review codes if the program is in place.
The vaccine-specific diagnosis is linked to each immunization line, and the place of service and modifiers for telehealth are kept up-to-date for all payers, so that the ancillary billing gets it right from the start.
UControl Primary Care Medical Billing Services
There are four coverage tracks that exist within a primary care claim, each one with its own adjudication logic. Office Visit payment is based on documented cognitive work. Wellness exam payment is on the benefit code that the problem visit cannot access. Care management is adjudicated on a monthly clock, and the ancillaries get paid only when the diagnosis coded in the claim matches the service. Get the logic of each one wrong, and the revenue is gone before the remittance comes.
E/M · G2211-Evaluation and management visits
Office and outpatient E/M services at 99202-99215 make up the bulk of primary care, and with the 2021 CPT changes the determination of level will be based either on the level of medical decision making or the total time spent on the visit. Still using the older coding guidelines, practices will downcode themselves, coding a 99213 when the complexity warrants a 99214. G2211 is an add-on to capture the complexity of the visit that is serving as the continuing focus on the health of the patient. Medicare will pay it independently starting January 2024 but practices are not adding it.
We will code the eligibility of G2211 based on the relationship recorded in the note rather than on the template check box, and we will verify the modifier 25 coding according to each payer's policy before submitting the claim. The complexity involved in a primary care visit is no longer the "unbillable line.”
Chronic and principal care management
Care management provides revenue from a primary care panel that does not typically bill for it in its practice. Chronic Care Management involves billing a patient who has two or more conditions and will have such conditions for at least one year: 99490 for the first twenty minutes of clinical staff time, 99439 for additional twenty-minute intervals of time up to the monthly limit. It is time-dependent, and hence it is either documented or not, because of minute tracking. Principal Care Management involves billing a patient based on a single condition, unlike CCM with two or more: 99424 and 99425 for physicians’ time and 99426 and 99427 for the clinical staff time per month.
With every Medicare patient, we place them into the program that best suits their clinical picture and meets all concurrency rules; we maintain documentation each month that proves the service and guarantees reimbursement after an audit.
Chronic and principal care management
A greater percentage of the money spent by the provider as far as primary care goes is riding on the quality measure than on the fee schedule, and the quality measure cannot be fulfilled unless the code that validates the fulfillment of the measure is found on the claim. If a screening is performed but not documented in codes, or the numerator is documented in the record but not on the claim, then the quality measure has been failed and the reimbursement reduced accordingly.
Why Primary Care Billing Is Complex Than Others
Primary care is just an illusion on paper. The visits are short and regular, and the coding looks very easy, but this illusion is exactly what erodes the bottom line. The reality is frequent, volatile, and merciless: every mistake in coding loses some money for each visit and thousands of visits per month, without any visit being denied enough to arouse any suspicion.
| Factor | Description |
|---|---|
| The 2021 E/M changes | The determination of level of complexity is based on the medical decision making or time. Coding strategies have been developed based on exam bullet lists, which actually lead to undercoding and lost income. |
| Modifiers usage | Modifiers 25, 59, and 24 apply to the coding of visits within the same day and global period. The incorrect use of modifiers leads to denials, while forgetting about them results in losing money. |
| Differences between payers' policies and NPP requirements | There are differences among payers with regard to preventive and problem codes and prolonged visits, while different rules apply to non-physician practitioners and incident-to guidelines. |
| Ensure proper UControl coding for every payer according to their | requirements and documentation policy, so that clean claims go out and the practice earns money from high-volume primary care patients. |
How UControl Billing Runs Your Primary Care Revenue Cycle
The entire cycle of your assertions goes through one responsible process from the moment of checking eligibility until performance reporting. Every step was designed in order to track the revenue as well as any mistakes made by primary care.

Eligibility and benefits verification
prior to the visit

Prior authorization
in case it is required from the payer or the service

Charge capture and code selection
G2211 and preventive capture included

Modifier and NCCI review
25, 59 and 24 discipline covered

Clean submission
of 837P claim form and clearinghouse scrubbing

Denials management
and appeals for those that resist payment

Posting of ERA 835
and reconciling to deposit
As coding, eligibility, submission and denials processing happen in one office and under one standard, then any problem detected during posting gets to the right people without handing over the case to another company.
Common Primary Care Billing Codes We Work With
Primary Care occurs in office visits, prevention, care management, immunizations, and telemedicine on a daily basis. Right coding not only codes the procedure but passes through the Medicare, Medicaid, and Commercial reviews. These are the family that gets you reimbursed.
| Code | Description |
|---|---|
| 99202-99205 – Office Visit and E/M 99212-99215 – Initial patient visits, levels 2 to 5 G2211 – Follow-up patient visits, levels 2 to 5 | Add-on code for complexity of visit for longitudinal primary care. The level depends either on the decision-making or total time since 2021 CPT modification. G2211 is the add-on code most practices underestimate. |
| 99381-99387 – Preventive and wellness 99391-99397 – New patient preventive visits by age G0438 / G0439 – Established patient preventive visits by age G0402 – Medicare AWV, initial and subsequent IPPE, first 12 months of Part B | Preventive is billed apart from problem-oriented care. Modifier 25 lets a same-day problem E/M be paid alongside the wellness service when documented distinctly. |
| 99490 / 99439 - Care Management 99491 / 99437 - CCM, Clinical Staff Time & Add-On 99487 / 99489 - CCM, Physician Time & Add-On 99424-99427 - Complex CCM 99495 / 99496 - PCM, One High-Risk Condition Transitional care post-discharge | CCM requires at least two chronic conditions whereas PCM is for one high-risk condition. Primary Care Specialties We Serve
Many types of practices offer primary care, each with its own coding, but still managed through one revenue cycle. We own the hub and steer the specialty depths into our team.
Billing services for internal medicine practices
Adult disease panels where the revenue flows via CCM, PCM and E/M leveling due to the number of comorbidities.
Billing services for family medicine practices
Care for all ages, combining well-child visits, adult preventive visits and problem visits in one day where modifiers 25 and immunizations link the claim.
Billing services for pediatricians
Well-child visits, vaccines administered and VFC and EPSDT requirements where diagnosis linking and age-based coding bring revenue.
Geriatric billing
We include women’s health services offered in a primary care environment and same day appointments, both involving 25 modifier specialties extensively.

What Your Practice Gains
True billing in primary care is in the figures that reflect your bottom line. Not in the slogans. This is what we manage to, and what a free audit checks for first.
When E/M levels reflect complexity of care, G2211 coding is included, care management programs are recognized, and modifier discipline is adhered to, first pass acceptance goes up and AR days come down. Revenue lost due to under-coding and lack of care management programs is revenue your practice earned but never received.

Primary care billing consists of all the steps involved in the complete process of managing revenues for longitudinal outpatient healthcare services that are first contact in nature. The high frequency of E&M encounters, prevention and wellness exams, and chronic condition management of primary care practices are converted to fully reimbursable claims using this revenue cycle process that includes all steps from eligibility through coding, claim submission, denial management, claim posting, and reporting.
The major codes used for primary care billing include Office/Outpatient E/M from 99202 to 99215 with G2211 for longitudinal encounter. For preventive coding, there are Medicare codes G0438, G0439, and G0402 and commercial codes 99381 to 99397. The care management codes include CCM (99490, 99439, 99491, 99487, 99489), PCM (99424 to 99427), TCM (99495,
Yes. In case the patient brings up a new or a worsened issue during the preventive or wellness visit, the problem-oriented E/M will be reported separately using modifier 25 in order to illustrate the significance and separately identifiable condition. The documentation should distinguish between the preventive services and problem services as payers pay attention to this combination and do not cover it if it is not distinguishable.
The billing of family medicine deals with patients of all age categories, thus, it combines well-child visits, vaccinations, preventive and problem-oriented services for adults based on modifier 25 and linking diagnoses. The billing of internal medicine focuses on adult chronic disease, thus, E/M coding based on the level of morbidity and care management programs such as CCM and PCM make up more income.
Yes. We bill the Annual Wellness Visit (G0438 and G0439) and the Initial Preventive Physical Exam (G0402), and we perform the complete care management package, which includes Chronic Care Management, Principal Care Management, Transitional Care Management, and Advanced Primary Care Management. Each individual is assigned to the appropriate program that complies with regulations and gets paid properly; and the subsequent monthly billing is performed.
The price will depend on the volume of your claims, payor mix, type of practice, and services being delegated. The price for primary care billing is normally given as a percentage of collection or flat rate per month; our pricing for your practice will depend on the results of the free billing audit, which will show the additional income we can generate for you.
