Explore Frequently Asked Questions
Nephrology billing covers chronic kidney disease care, end-stage renal disease (ESRD), dialysis, and transplant-related services. It involves separate facility and physician billing rules, including the ESRD Prospective Payment System (PPS) bundle, monthly capitation payments, specific modifiers, and Medicare Secondary Payer (MSP) rules. Small coding or documentation errors can quickly lead to denials or underpayment.
ESRD PPS is the bundled payment Medicare makes to the dialysis facility. It covers the treatment, equipment, routine labs, and most related drugs, and it is billed on facility claims (bill type 72X). MCP (CPT 90951–90970) is the monthly payment to the nephrologist for managing dialysis care, based on the patient's age and the number of face-to-face visits in the month.
When a patient with employer group health plan (EGHP) coverage becomes eligible for Medicare due to ESRD, the EGHP stays primary for a 30-month coordination period, and Medicare pays secondary. After 30 months, Medicare becomes primary. Keeping the payer order updated is essential to avoid denials and payment recoupments.
- Dialysis codes: 90935 and 90937 (hemodialysis), and 90945 and 90947 (other dialysis, such as peritoneal).
- Monthly ESRD services: 90951–90970.
- Common modifiers: -G6 (fewer than six dialysis sessions in a month), -AY (item or service not for ESRD treatment), -25 (separate E/M service), and -59 or -XE (distinct service).
Routine ESRD labs and most renal-related drugs are included in the ESRD PPS payment. Labs or drugs that are medically necessary and not related to ESRD treatment may be billed separately with the -AY modifier and supporting documentation. Correct use of this modifier is important, because misuse can trigger denials or audits.
Where required, we handle eligibility verification and prior authorization for services such as vascular access procedures, biologics, and transplant-related care. Our certified coders review documentation against Medicare LCDs, NCDs, and payer policies before claims are submitted. If a claim is denied, we correct and resubmit it or file an appeal promptly.
Common issues include incorrect CKD stage coding (ICD-10-CM N18 series), missing documentation for MCP face-to-face visits, incorrect coding of vascular access procedures (such as 36901–36909), and outdated primary insurance during the MSP period. Catching these errors before submission reduces denials and rework.
Onboarding typically takes 2 to 4 weeks. We work inside your existing EHR and clearinghouse, review your current billing workflow, and set up claim-scrubbing rules. The transition happens without disrupting your daily practice operations.
