Staying Up to Date With Government Offers and Incentives
The spread of infectious diseases is one of the areas of medicine that the government pays the most attention to curtail and eliminate before they spiral out of control. Vaccinations and treatment protocols need to be effectively implemented on a priority basis when an epidemic is identified.
Staying informed and updated with the regulatory changes in healthcare billing, particularly those issued by the Centers for Medicare and Medicaid Services (CMS), is important.
It is enough for healthcare providers to focus on delivering exceptional patient care to stay on top; spending time on figuring out how to do billing and coding for each new thing that frequently comes along is just too much distraction.
That’s why outsourcing your billing services to a trustworthy infectious disease medical billing company is the most viable option. ID billing relies heavily on time-based E/M coding, complex multi-system cases, and long-term care such as outpatient IV antibiotic therapy (OPAT). It also requires organism-specific ICD-10 coding, antimicrobial resistance codes (Z16 series), and careful handling of same-day procedure bundling.
OPAT billing has two parts:
- Administration: CPT 96365 (first hour), 96366 (each additional hour), and 96367 (additional sequential infusion of a different drug), when given in a clinic or infusion suite.
- Drug supply: The J-code for the specific antimicrobial, with modifier JW for discarded drug amounts or JZ when there is no waste.
Home infusion is usually billed by the home infusion pharmacy or agency, not the ID office. Commercial payers often use per diem S-codes (e.g., S9494–S9504), while Medicare home infusion rules are different. The ID physician bills the E/M visits used to monitor therapy.
For time-based visits, document the total time the billing provider personally spent on the date of the encounter, including both face-to-face and non-face-to-face work, such as reviewing cultures and labs, coordinating care, and documenting. Time spent on separately reported services is not counted. Prolonged services are billed with CPT 99417 (office/outpatient) or 99418 (inpatient), while Medicare uses G2212 and G0316 instead. Each requires the time to exceed the base visit's time threshold.
- Systemic infection: The underlying sepsis code first (A41 series), e.g., A41.9 for unspecified organism, A41.01 for MSSA, or A41.02 for MRSA.
- Severe sepsis: R65.20 (without septic shock) or R65.21 (with septic shock).
- Organ dysfunction: Additional codes for each documented failure, such as acute kidney or respiratory failure.
Codes must be supported by the provider's documentation.
Z16 codes (Z16.10–Z16.39) identify resistance such as ESBL (Z16.12) and vancomycin resistance (Z16.21, used for VRE). They are reported as secondary codes after the infection code. They support the complexity of care, the choice of higher-cost antimicrobials, and the severity of the case. Some organisms, such as MRSA, already carry resistance in their own code (e.g., B95.62), so an extra Z16 code is not needed.
Procedures such as abscess aspiration (CPT 10160) or central line placement (CPT 36556) require Modifier 25 on the E/M code. The E/M must be significant and separately identifiable, and must go beyond the decision to perform the procedure and the usual pre-procedure work. Document the two services separately.
Our certified coders review documentation to confirm that diagnoses and culture results are supported by the provider's notes, check time-based E/M documentation, and scrub claims against NCCI edits and payer rules before submission. We also track denials and follow up promptly, so your claims are paid faster.
We work with commonly used EHR/EMR platforms, including Epic, Cerner (Oracle Health), athenahealth, eClinicalWorks, NextGen, and AdvancedMD. Our team can work inside your existing system for claim submission, payment posting, and patient billing, with minimal disruption to your workflow.
It depends on the payer. Medicare does not pay consult codes (CPT 99242–99245 and 99252–99255), so ID physicians bill a new or established patient E/M instead. Many commercial payers still accept consult codes, so we verify each payer's policy before billing.
