Why Choose a Specialized Hospital Billing Company
The claims fail in hospitals in ways they don’t in clinics. That is why we constructed our process to avoid those failure modes.
81 Form locators
We claim the whole institutional claim. This includes condition, occurrence, and value codes a professional biller will never touch.
2 Parallel payment systems
We apply IPPS and OPPS algorithms to the same patient group, assigning the encounter to one of them even before applying any code.
§412.3 Status decided in advance
We document two-midnights criteria at admission. That is where it applies, not during an audit.
2026 Up-to-date with the regulations
We file under this year’s IPPS and OPPS final rules and modify if CMS modifies them, not from last year.
Common Hospital Billing Problems and How We Route a Denial
Status denials are followed by the utilization review record. Medical necessity denials are followed by the diagnosis that supports the necessity. Coding denials are followed by the document that supports the coding. In case of traditional Medicare, a redetermination should be completed within 120 days from the date of the original determination. Each new level begins with the previous one.
We capture those losses that do not come with a denial notice. The payments that are done below your contracted rate are posted as underpayments, not rejections. This is how we check those payments against your contract so the underpayment is captured and the contract will not be closed silently.
Recoups are considered on par with the front-end denials. The payer may reverse the payment of the claim that was paid several months ago due to the status or coding review. We reply to recoups with the documents that we have. The paid claim is captured, but not reversed silently.Late filing is the only denial that cannot be argued back, which is why we monitor it as a deadline. Every insurer has its own deadline for filing claims, and commercial deadlines may even be stricter than those of Medicare. We time our claims carefully while they are still pending.
How We Route A Denial
| Reason | Source | Correction |
|---|---|---|
| Incorrect status | Two-midnight expectation not documented | Record the rationale at admission |
| Revenue-to-HCPCS mismatch | Outpatient line missing a valid HCPCS code | Pair each revenue line with a procedure code |
| Type of Bill error | Wrong facility or frequency code in FL4 | Match the code to the encounter |
| Medical necessity | Diagnosis does not support the service | Confirm the diagnosis before submission |
| Authorization | Scheduled service lacks prior authorization | Verify authorization before service |
| Discharge status | Wrong FL17 code on a transfer | Confirm disposition at discharge |
| Coding specificity | Unspecified ICD-10 where a specific code exists | Assign the most specific code supported |
| Timely filing | Claim filed after the payer deadline | Submit within each payer's limit |
What UControl Hospital Billing Services Include
There are seven steps in the process of generating hospital revenues. With UControl, all steps happen to be taken care of. The error that occurs during the time of registering will not turn into a denial after three weeks. When a patient gets admitted into a hospital, the facility claim is generated using the UB-04. However, another professional claim is also generated in the name of the physician. In our work, we take care of the facility claim because it includes codes of payment that cannot be found on the professional claim.
Access and eligibility
Coverage and benefits are checked when making the appointment and once again prior to treatment; authorizations are obtained for every scheduled procedure.
Status assignment
Inpatient and outpatient status are determined based on the two-midnight expectation, referring to physician advisors in cases when the determination is uncertain.
Charge capture
Charges are reconciled to the medical record. Unbilled items are found prior to the claim filing.
Facility coding
By certified coders, ICD-10-CM and ICD-10-PCS are assigned for inpatient claims while HCPCS/CPT codes are assigned for outpatient claims.
Claim submission
The 837I claims are scrubbed for revenue-to-HCPCS pairing and Type of Bill, and transmitted using your clearinghouse.
Payment posting
835 remittance is posted using your contracted rate and alerting you if a payment is lower than the agreed amount.
Denial and appeals
Denials are grouped based on the cause of denial and each is submitted to the appropriate appeal level timely.
How the Two-Midnight Rule Affects Inpatient Billing
The difference between the two is the biggest amount of money you can ever hope to recoup from the majority of facility claims that are outstanding. We view patient status as a front end decision rather than a billing issue
Our reviewers review the two midnight criteria of 42 CFR §412.3 at admission. If the evidence suggests that an admission requires two midnights, we include that rationale before the claim is even built. In cases where the admission gets truncated, we use the condition code 44 and re-bill as outpatient.
It is especially relevant when dealing with Recovery Audit Contractors who audit the short inpatient admissions using the look back period of six months. The claim we file already has all the information an auditor asks for. The peer-to-peer review that we perform for the Medicare Advantage denials usually overturns them.
The documentation we produce is precise. The admitting physician’s documentation of the clinical basis for the expected two midnights’ stay is confirmed. It is ensured that the order specifies the inpatient status, but not the observation one. The inpatient admission without such documentation is exactly what the auditor recovers. There are specific guidelines that govern the observation status, and we follow them strictly. The patient who is kept under observation will be an outpatient from the perspective of OPPS, but he/she will not be an inpatient admission.
What the status decision controls
| . | UControl Billing |
|---|---|
| Payment system applied | IPPS or OPPS |
| Governing rule | 42 CFR §412.3 |
| Reclassified-stay flag | Condition code 44 |
| RAC look-back | 6 months |
| MA appeal path | Peer-to-peer |
| Retro appeal window | Closed Jan 2, 2026 |
Why is this a front-end job?
The standard window for appeal by Medicare to reverse status determinations retroactively closed on January 2, 2026. Status determination at admission is the only guaranteed protection; that is why we put it before coding, not after denial.
IPPS and OPPS: The Two Systems We Code Against
There are two payment methods within the Medicare program for hospitals, and they both have the same department with different billing processes. For inpatients, we assign the appropriate MS-DRG for the diagnosis, procedures, and severity contained in the medical records. Since one complication/comorbidity would change the weight of the MS-DRG assigned, our coding staff makes sure that we have documented evidence of the appropriate weight of the case. Before we finalize the assignment of the primary diagnosis, we confirm the DRG assignment. On the outpatient side, we assign each procedure a corresponding APC and look at the status indicator. A bundled service that is billed as separately payable is an example of a denial avoided by coding.
| Factor | Inpatient (IPPS) | Outpatient (OPPS) |
|---|---|---|
| Unit paid | Per admission | Per service |
| Classifier | MS-DRG | APC |
| 2026 update | 2.6% | 2.6% |
| Base figure | $6,752.61 | $90.970 |
| Outlier trigger | $40,397 | $6,225 |
We Verify UB-04 Claim Form and 837I Before Submission
The UB-04, formally the CMS-1450, is the standard institutional claim form. It contains 81 form locators. Its electronic equivalent under HIPAA is the 837I transaction, which carries the same data.
The patient discharge status is checked for every transfer. The claim is submitted with 837I with all data in the appropriate field. If there is an error from the clearing house for the locator, the mistake is corrected at the originating point and not resubmitted blindly.
The revenue code discipline is another one that should be considered, and the codes are determined by the departments. The emergency room, operating room, pharmacy, and laboratory have their own revenue codes. An incorrectly coded revenue code charge may still be accurate in dollars.
Fields we verify before submission
Type of Bill (FL4)
Facility and frequency code matched to the encounter
Condition codes (FL18 to FL28)
Code 44 applied on any reclassified stay
Discharge status (FL17)
Disposition confirmed to protect transfer payment
Revenue and HCPCS (FL42 to FL44)
Each outpatient line paired with a procedure code
Principal diagnosis (FL67)
Verified before the MS-DRG is set

How We Work With Your Revenue Cycle Team
The revenue cycle for a hospital organization comes with its own people, processes, and legacy. Instead of having you adapt our solution, we work within the context of what you have. Our personnel work within your EHR system and patient accounting system, subject to your control access.
Our communication follows a consistent cadence. Your account has an account leader who understands your payer mix. We conduct a regularly scheduled review of denials, underpayment issues, and outstanding appeals according to the schedule that your organization has established. Communication goes directly to an individual, not to a ticketing system.
Coverage will vary depending on the volume that you have. It can range from a full cycle at the hospital level to a particular phase such as inpatient coding and denial management. The scope of coverage will be known before work commences.

Revenue Cycle Metrics and Reporting
We measure against what is already being measured on a hospital financial dashboard so that we do not need a whole new language of metrics in order to be able to measure ourselves. Each of these metrics is linked back to what has been discussed above. Clean claim ratio is measured based on the edits performed prior to billing. Denial rates per cause reveal if it is a result of either status issue, coding issue, or authorization issue. Case mix index varies depending on the quality of coding.

Clean claim rate
Share of claims paid without rework on first submission.

Denial rate by reason
Denials grouped by cause to show the source, not just the count.

Days in A/R
Average time a facility claim remains unpaid.

Case mix index
Average DRG weight across inpatient discharges.

Denial overturn rate
Share of appealed denials that reverse.

Net collection rate
Share of contracted revenue actually collected.
Our Credentials
We hold the certifications that payers and auditors look for, and our coders carry the right credentials. It’s not the most exciting part of the work, but it’s the part that keeps your claims on solid ground.

Hospital billing includes facility expenses from the UB-04 form while physician billing includes professional expenses from the CMS-1500 form. The hospital stay may require submitting both claims to the same payer.
Inpatient stays in Medicare are reimbursed by IPPS using MS-DRG method while outpatient services are reimbursed by OPPS using APCs method. The prospective payment method is set first and adjusted for classification.
Hospitals charge using the UB-04, officially referred to as CMS-1450. The electronic version of this document is 837I. This document has 81 form locators for revenue codes, condition codes, and diagnosis codes.
According to the 42 CFR §412.3, the two-midnight rule is used in Medicare Part A where an admission is considered to be appropriate if the patient's stay will likely exceed two midnights.
CMS started to eliminate the Inpatient Only list in a period of three years and also started site-neutral payments. The hospitals are also required to submit median Medicare Advantage negotiated charges by MS-DRG which will help in calculating inpatient weights in FY2029.
The time limit for the Level 1 redetermination is 120 days from the day of the initial determination. Every other level of determination also has its deadline depending on the previous decision. Failing to meet the deadline means losing that particular level.
