Home health agencies bill Medicare on institutional UB-04 claims (TOB 032X) instead of CMS-1500. Payment follows the Patient-Driven Groupings Model (PDGM) in 30-day periods, based on OASIS data, clinical grouping, functional impairment, admission source, timing, and comorbidities.

The NOA (TOB 32A) tells Medicare a patient has started home health care. It must reach the Medicare Administrative Contractor (MAC) within 5 calendar days of the Start of Care. Late NOAs reduce payment by 1/30 of the 30-day rate for each day from the SOC date until submission.

Each PDGM 30-day period has a visit threshold, generally 2 to 6 visits. If an agency provides fewer visits than the threshold, Medicare pays per visit instead of the full period rate. We track visits against LUPA thresholds so managers can act before the period closes.

OASIS responses drive PDGM functional scoring and payment. Denials occur when OASIS, the Plan of Care (CMS-485), and visit notes contradict each other. We check that diagnoses, functional levels, and clinical notes match before the final claim is submitted.

A physician, NP, CNS, PA, or certified nurse-midwife must see the patient within 90 days before or 30 days after the Start of Care. The visit must relate to the primary reason for home health, and the records must support homebound status and skilled need.

Visits are reported in 15-minute units with revenue codes and G-codes:

    • G0299 / G0300: Skilled nursing (RN / LPN).
    • G0151 / G0152 / G0153: PT, OT, and speech-language pathology.
    • G0155 / G0156: Social work and home health aide.

We assemble the full record (orders, F2F notes, signed plan of care, OASIS) and respond to ADRs and TPE reviews before the 45-day deadline. We appeal denials with homebound and medical necessity documentation, and review 30/60/90+ day A/R regularly.

Onboarding usually takes 2 to 4 weeks. We connect with your EMR (HCHB, Axxess, WellSky, MatrixCare, or KanTime), set up clearinghouse workflows, and keep disruption to care and cash flow to a minimum.

The final claim (TOB 329) is submitted after the 30-day period ends or the patient is discharged, once the signed plan of care and orders are on file. Medicare's timely filing limit is 12 months from the through date.

Under consolidated billing, the agency bills for nursing, therapy, and supplies provided during a 30-day period, and outside providers generally cannot bill Medicare separately for them. Durable medical equipment (DME) is a major exception.
