Why FQHC Billing Is Different From Standard Medical Billing
Each FQHC claim is based on the qualifying encounter and its PPS encounter rate. Thus, payment is based on the entire visit rather than on several individual codes.
Our team processes FQHC claims under the encounter-based PPS model and not under the itemized fee-for-service that most billers use. Even a full visit with an E/M service, lab and procedure will pay one flat per encounter rate. This is the only factor that makes all FQHC claims unique.
That is why UControl Billing sees FQHC claims as a payment discipline and not a clinical coding process. Clinical coding takes place during the visit; however, payments are made at the encounter level and not at each individual line item level. A claim created using a fee-for-service method for PPS payer is erroneous.
Standard billing Itemized fee-for-service
Each service provided on a visit is coded and reimbursed on a separate line according to a fee schedule. Total reimbursement is the sum of individual codes for each service rendered.
FQHC · PPS PPS encounter rate
A qualified visit receives one all-encompassing per-encounter rate payment irrespective of how many services occur during that visit. The use of CG modifiers makes it qualify for such a payment.
Our team prices each FQHC qualifying visit at its PPS encounter rate. The payment therefore becomes an expression of the bundled methodology.
UControl Bills FQHC Visits Under The Prospective Payment System
This is the per visit, all-inclusive PPS rate applied to health centers as opposed to the fee schedule. The PPS rate is pegged to national rates based on the Medicare Physician Fee Schedule. A qualifying visit is paid the PPS rate regardless of the services performed during the visit being one or many.
Our coders are in line with the status quo in terms of the FQHC PPS codes set. The CMS has not reviewed the core FQHC PPS G-codes set since 2017. Payment rates and policies continue to flow via the yearly rule making cycleVerify current year vs CMS. The billing solution follows the updates annually although the encounter logic remains the same.
The reason why PPS billing is demanding is the determination of the qualifying visit. A visit has to qualify as a visit before it gets paid the PPS rate. Non-qualifying services will not be paid separately. We validate the visit against the qualifying criteria prior to billing the PPS rate.
Where FQHC Encounters Go Unpaid Without Specialist Coding
Each FQHC encounter gets coded with the precise G-code, revenue code and CG modifier that will pay for the encounter. These are the areas where a generalist coder most frequently causes a loss of encounter rate for the health center. One missing modifier changes a qualified encounter to a denied claim. These are the three deficiencies which cause most encounters money that FQHCs lose prior to payment. With us, each one is handled as a routine coding process rather than a correction post denial.
| Modifier | Discription | UControl Billing |
|---|---|---|
| CG modifier The modifier which leads to payment | CG modifier is reported on the line for the primary reason for the encounter. Without CG modifier, the encounter does not qualify for PPS payment and gets denied. | CG modifier must be placed on all qualifying encounters prior to submission. |
| G0466 through G0470 The appropriate visit G-code | Each visit has a specific payment G-code based on a medical or preventive or mental health encounter. When the visit is coded under an incorrect G-code, it will be paid under the wrong rate or not paid at all. | Corrects the qualifying-visit G-code based on the encounter documentation. |
| Rev code 0521 + The corresponding revenue code | The revenue code determines the location of the encounter. A clinic visit utilizes 0521 and other locations use different revenue codes. The incorrect revenue code will cause the claim to be delayed by the payer. | Ensures that the revenue code matches the actual location of the encounter. |
Medicaid Wraparound Payments And Multi-state PPS Variation
Our system will help keep track of and reconcile the Medicaid wraparound payment to each health center. This is the amount that the state should pay back to the FQHC as they have not been paid their full PPS rate by the managed care organization. The federal government assures that the FQHC should be paid at least the PPS rate. The difference in the MCO payment and the PPS rate is reconciled separately.
The process of reconciliation is done on a quarterly basis and is completely dependent on the accuracy of the encounter data reporting process. It would not be possible to calculate the wraparound payment without proper encounter data being available.
Encounter billed to the MCO
The FQHC bill the MCO for the qualifying visit.
The MCO pays less than the PPS rate
The payment by the managed care plan is often less than the entire FQHC PPS rate.
The state makes the wraparound
The state Medicaid program provides the extra amount to ensure that the center gets the entire PPS rate.
Our team takes care of this state by state because the policies are different. The Medicaid PPS and alternative payment methodologies are handled individually in each state. Encounter criteria and requirements and the process of reconciliation differ state by state. This is where having nationwide coverage means working according to individual states’ policies and not according to one national policy. Expanding to another state implies learning the new terminology of wraparounds.
We only bill FQHC telehealth encounters under the current originating-site and code rules. Claims then reflect the active telehealth policy rather than a lapsed version.
UControl Telehealth Billing For FQHCs
Our billers process telehealth claims by FQHCs using the unique billing requirements of telehealth that distinguish it from a traditional face-to-face encounter. Telehealth requires a different revenue code and originating site treatment as compared to a traditional clinic encounter. This is one of the most updated areas when it comes to FQHC billing.
We keep track of the flexibility rules applicable to telehealth during each cycle of the rule process. Telehealth encounters billed under old guidelines is one of the commonest causes of denial of claim. The process of staying current with the rules and originating site treatment is part of it.
Credentialing And Payer Enrollment For FQHC Providers
Our program handles the credentialing process which ensures the FQHC encounters are paid rather than being denied up front. The biggest reason for denial of FQHC encounters is due to credentialing problems. High turnover rate among providers and locums and also multi-site licensure increase the enrollment work burden.
The process regards enrollment as an ongoing one because the provider list of the FQHCs keeps changing constantly. The encounter from an unenrolled new provider would never get billed. Every payer has its own enrollment timeframe to be met by the center.
Credentialing backlog converts clinical availability into non-billed visits. Each day that a provider is unenrolled is a day where their visits go unbilled. UControl Billing works enrollment before the date of service to prevent this issue. Our experienced team does FQHC credentialing and payer enrollment along with billing. Thus, provider visits remain billable right from day one of their services
Provider enrollment
Each provider is enrolled with Medicare, Medicaid, and contracted payers before billing the encounters.
Multi-site licensure
Providers who provide services to multiple FQHC sites have their credentials verified by site as needed by payers.
Locum coverage
Providers working temporarily are managed, so as to avoid any lapses in coverage that result in billable visits.
Revalidation
Revalidation of enrollment happens on payer cycles to ensure that there are no lapses in billing privileges.
Accuracy of the encounter becomes an important compliance concern, as far as UDS reporting by an FQHC to HRSA is concerned. If there is an error in the UDS report due to wrong information contained in the billing system, then it is our billers who will ensure that the billing system does not get corrupted. They synchronize FQHC encounters with UDS reports and Section 330 requirements. The compliance data is based on the same billing data as the claims.
Compliance, UDS Reporting, HRSA, and The Sliding Fee Discount Schedule
We ensure that the data used in FQHC compliance reporting to HRSA is accurate. Compliance reporting includes the reporting of annual encounter and operations data to HRSA. Accurate billing and encounter data go into the report; thus, there are consequences for inaccurate billing besides revenue loss.
Our billing workflow fits the grant structure that qualifies a facility as an FQHC. There is the grant program contained in Section 330 of the Public Health Service Act which makes the FQHC qualification possible. Section 330 comes with the sliding fee discount schedule which must be implemented by health center grantees.
Look-Alikes are facilities that are similar to FQHCs. They have been certified by HRSA and are PPS-eligible. However, they do not qualify for Section 330 grants. Our team will bill both types under the same PPS framework.
FQHCs, CHCs, Look-alikes, and Related Safety-Net Providers
UControl supports all the safety net providers who are billed through the FQHC and FQHC equivalent encounter billing models. Even though the program designation changes, the billing process remains the same.
FQHCs and CHCs
Section 330 grant recipient clinics billed using the FQHC PPS encounter model.
FQHC Look-Alikes
HRSA certified and PPS eligible clinics which are not funded through the Section 330 grant
Tribal health programs
Tribal clinics billed using the FQHC equivalent encounter billing model
School-Based Health Centers
School-based health centers billing their encounter using the same PPS mechanics
Homeless Health Programs
Homeless health programs providing encounter-based safety-net health care.
Migrant Health Programs
Migrant and seasonal health programs billing under FQHC equivalents.
UControl provides a single FQHC encounter model regardless of the type of program and specialty a center may offer. The billing remains constant as each clinical service maintains its own coding.
Outsourcing Versus In-House FQHC billing
Billing in an FQHC requires individuals who understand both the encounter logic of the PPS and the reconciliation of wraparound along with multi-state Medicaid guidelines simultaneously. This combination of skills is present in an internal group with turnover while very few billers have this expertise.
| Function | In-house billing team | Outsourced to UControl Billing |
|---|---|---|
| PPS encounter expertise | Rests on individual staff familiar with FQHC encounter logic. | A team that bills FQHC PPS encounters daily across health centers. |
| Wraparound reconciliation | Quarterly reconciliation competes with daily claim work. | Wraparound tracked and reconciled as a standing function. |
| Multi-state Medicaid | Each new state adds rules staff must learn on their own. | State-specific PPS and wraparound rules mapped at onboarding. |
| Coverage continuity | Turnover in a specialized role leaves encounters unbilled. | Encounters billed continuously without single-person dependency. |
| Cost structure | Fixed salaries plus training for a hard-to-staff position. | Scales with encounter volume rather than fixed headcount. |
How UControl Billing Supports FQHC Revenue Cycle Management
Triggers for PPS include the use of G-code and CG modifier in each qualifying visit. This information is used to reconcile the Medicaid wraparound and to prepare the UDS report. It’s important to make sure that there is active enrollment prior to billing by the provider because front-end accuracy is essential. Everything goes through our billing workflow in claims scrubbing, denials management, and medical coding.
PPS encounter and G-code accuracy
Each encounter is billed using the correct payment G-code for the encounter, with the CG modifier for the PPS rate. The qualifying visit criteria are validated prior to filing.
Medicaid wraparound reconciliation
Wraparound shortfalls between MCO payment and the PPS rate are calculated and reconciled. Data for encounters remains scrubbed to ensure the state pays the total shortfall.
Claims submission and scrubbing
FQHC claims scrubbing occurs for encounter logic and for the payer’s claim submission process. See claims submission and scrubbing.
Patterns of FQHC denials are addressed at their core through issues such as no CG modifier and no supporting qualifying visit. Denial management details.
Provider enrollments are scheduled to beat start dates so encounters can remain billable. Credentialing service details.
AR Recovery and Aging
Facility-level and wrap-around related aging buckets are monitored and resolved prior to expiration of filing limits. Accounts Receivable recovery details.
Encounter coding is consistent with PPS G code sets and clinical notes for each visit. Proper coding helps avoid recording and denying encounters in the process.
FQHC Billing Software and EHR Compatibility
We operate in the context of the current practice management system and EHR environment at the FQHC and do not require a different platform. The billing process is built in such a way that it fits into the current stack used by the health center. Our billing process is concentrated on the handoff precision of information between the clinical record and the bill. We reconcile the handoff of the encounter from the EHR to the claims file in the center’s existing systems.
Clinical / encounter EHR and PM systems
Data from encounters and providers and visits is in the health center's clinical and practice management platforms. This is the data that establishes what can be billed.
Integrated care Dental and behavioral systems
In most cases, FQHCs have separate dental and behavioral systems. Same day encounters require integration based on one visit date.
Claims / clearinghouse Billing and clearinghouse solutions
The clearinghouse reviews and routes the FQHC encounter claims to each payer. Edit checks identify any mistakes regarding formatting and eligibility before submissions.
Why Health Centers Choose UControl Billing
FQHC billing is a specialized form of facility reimbursement within revenue cycle management, not just another keyword applied to a general service. UControl Billing uses the intricacies of how to get the FQHC encounter reimbursed at the PPS rate.
Encounter-based PPS
Each qualifying encounter is assigned the PPS payment G-code and CG modifier. UControl Billing bills the encounter through PPS and not fee-for-service.
Wraparound reconciled
The gap between MCO reimbursement and PPS is reconciled. Clean encounter data helps to make sure the state reimburses the wraparound owing to the center.
50-state coverage
UControl Billing assists FQHCs throughout the country. The Medicaid PPS and wraparound policies of each state are mapped during onboarding and not just assumed.
HRSA-friendly
Encounter data is maintained accurately for UDS and Section 330 reporting. Credentialing and denial and AR processing functions are performed by the same team.
Medical Billing Certifications and Compliance Standards
We hold the certifications that payers and auditors look for, and our coders carry the right credentials. It’s not the most exciting part of the work, but it’s the part that keeps your claims on solid ground.
Explore Frequently Asked Questions
FQHC billing is the coding and submitting of encounter-based claims from health centers operating on the Prospective Payment System. The visit will be paid using an all-inclusive per-encounter rate rather than fee-for-service. The management of such billing together with the Medicaid wraparound reconciliation and HRSA compliance data is done by UControl Billing.
The FQHC PPS refers to the fixed payment per-visit that is set for health centers. The amount is calculated based on national payments associated with the Medicare Physician Fee Schedule. A qualifying visit will pay at that rate irrespective of the number of services performed during the visit.
The Medicaid wraparound payment pays the difference between the full FQHC PPS rate and the amount underpaid by the managed care plan. The state Medicaid program is responsible for making that payment. Federal statute assures the FQHC receives at least its PPS payment. The discrepancy is settled apart from the regular process, typically quarterly, and using correct encounter information.
G0466 and G0467 are the codes for FQHC qualifying new and established medical visits. G0468 applies to a preventive visit or wellness visit. G0469 and G0470 are the codes for mental health visits and each has a CG modifier which initiates PPS payments.
When the same practitioner visits the same patient with the same diagnosis on the same day, it usually qualifies as one visit. The only exception is if there is a qualified medical visit as well as a qualified mental health visit on the same day.
Both are HRSA certified and qualify to use FQHC PPS billing. An FQHC qualifies for Section 330 funding under the Public Health Service Act, while an FQHC Look-Alike does not qualify for that funding.
Yes, our FQHC Billing services are available in all 50 states. Medicaid PPS and wraparound are processed on a per state basis, which is why we map out the process per state during onboarding.
