Explore Frequently Asked Questions
Chiropractic medical billing includes insurance eligibility and benefits verification, prior authorization when required, charge entry, medical coding, claim submission, claims scrubbing, modifier review, denial management, payment posting, and accounts receivable (A/R) follow-up. Billing support can also cover spinal manipulation, applicable therapy services, and Medicare-specific documentation requirements.
Chiropractic billing requires careful attention to payer-specific coverage rules, medical necessity, documentation, and treatment classifications. Medicare, in particular, has specific requirements for spinal manipulation, including active treatment of spinal subluxation and proper use of the AT modifier. Private payers may have additional coverage, authorization, and visit-limit requirements.
The primary chiropractic manipulation codes are:
- 98940: Spinal manipulation, 1–2 regions
- 98941: Spinal manipulation, 3–4 regions
- 98942: Spinal manipulation, 5 regions
- 98943: Extraspinal manipulation, 1 or more regions
For Medicare, coverage is generally limited to spinal manipulation to correct subluxation; 98943 is not covered under standard Medicare chiropractic coverage.
The AT modifier (Acute Treatment) is required on Medicare claims for CPT 98940, 98941, or 98942 when the chiropractor provides medically necessary active or corrective treatment for acute or chronic spinal subluxation. It should not be used for maintenance therapy. Claims without the required AT modifier may be denied.
Depending on the payer and the provider's scope of practice, certain therapy services may be billed separately when they are covered and medically necessary. Modifier 59 or an appropriate X modifier should only be used when the services meet NCCI requirements for being distinct, such as a separate encounter or different anatomical site. It should not be used simply to bypass a bundling edit.
An E/M service may be separately reportable when the payer allows it and the documentation supports a significant, separately identifiable evaluation. Modifier 25 may be required when appropriate. However, standard Medicare chiropractic coverage is limited to spinal manipulation for subluxation, so E/M coverage depends on the applicable Medicare policy or other payer rules.
Common denial causes include missing or incorrect AT modifiers on applicable Medicare claims, insufficient documentation of subluxation, incorrect diagnosis coding, medical-necessity issues, authorization problems, payer-specific coverage limitations, and incorrect use of modifiers. Reviewing documentation and payer requirements before submission can help reduce avoidable denials.
Medicare requires documentation of the precise level of spinal subluxation. Subluxation may be supported by imaging or physical examination using the PART criteria. For physical examination, at least two of the four criteria are required, with at least one being asymmetry/misalignment or range-of-motion abnormality. The subluxation should be reported as the primary diagnosis, with the related neuromusculoskeletal condition reported as appropriate.
Personal injury and workers' compensation billing may require claim and lien tracking, medical-record submission, payer or attorney communication, and applicable fee-schedule review. For cash-pay services, practices should use clear patient financial agreements and appropriate superbills. Medicare ABN requirements apply only when applicable to Medicare-covered beneficiaries and services.
Yes. Chiropractic billing services can work with compatible EHR and practice management systems to support charge capture, documentation review, claim submission, payment posting, and reporting. Specific integration capabilities depend on the software platform and available interfaces.
