Allergy billing separates allergen extract preparation (CPT 95165) from injection administration (95115/95117). It also involves per-test unit counting for skin testing (95004, 95024), high-cost biologic authorizations, and frequent same-day E/M and procedure claims that need Modifier 25
They are billed separately:
- Preparation (CPT 95165): Billed per dose of antigen extract prepared. For example, a 10 mL multi-dose vial with 1 mL doses is billed as 10 units.
- Administration (CPT 95115 / 95117): 95115 is for a single injection, and 95117 is for two or more injections on the same day.
Modifier 25 is added to the E/M code (e.g., 99213 or 99214) when the provider performs a significant, separately identifiable evaluation on the same day as skin testing or an injection. The documentation must show the visit went beyond a routine pre-injection check. It is not added to the procedure code.
Biologics and IVIG commonly require prior authorization. We first confirm whether the drug falls under the medical or pharmacy benefit. Then we verify payer criteria, compile clinical evidence (such as IgE levels, eosinophil counts, and prior treatment failures), submit the request, and track re-authorizations to help avoid treatment interruptions.
- Testing units exceeding payer limits (e.g., CPT 95004).
- Incorrect dose counts for extract preparation (CPT 95165).
- Missing Modifier 25 on same-day E/M visits.
- Missing documentation of medical necessity, or ICD-10 codes (e.g., asthma or allergic rhinitis) that don't support the service.
Percutaneous (prick or puncture) tests are billed with CPT 95004, and intradermal tests with CPT 95024. Both are billed per test, so 40 prick tests are billed as 40 units of 95004. Many payers set unit limits, and each test must be documented.
Yes. Our certified coders scrub claims against payer-specific unit limits and coverage policies (LCD/NCD) and review antigen preparation logs before submission. We also follow up on denials promptly, so your claims are paid faster and with fewer rejections.
Onboarding usually takes 2 to 4 weeks. We connect to your practice management system and EHR (such as athenahealth, eClinicalWorks, ModMed, or Tebra), set up clearinghouse rules, and transition billing with minimal disruption to patient care and cash flow.
Oral food challenges are billed with CPT 95076 for the first 120 minutes and 95079 for each additional 60 minutes. Documentation must show the start and stop times, the foods given, and the patient's monitoring and reactions.
